COMPREHENSIVE PHYSICAL EXAMINATION REQUIREMENT

By state law, our school district requires that all athletes submit a new comprehensive physical examination each year that they participate in school sports. 

This form must be completed and signed by the athlete's family physician and dated after June 1 of the current year.
