REQUIRED DOCUMENTS

You must have the following documents with you at the time of registration:  

 

 

Proof of Residency – See attached form for acceptable documents.

Identification – for yourself
Original immunization record for the child.
Proof of Age – original birth certificate, baptismal certificate, hospital certificate, visa, green card.
 
Any specialized reports such as psychologicals , psychiatrics, speech and language evaluations.
Are you the biological parent? Please provide identification.
If you are single, divorced, or a separated parent; do you have legal custody? If yes, we will need a copy of the custody paper for the child's file.
Do you live at the same address as your child? If no, a “Pre-affidavit Statement” form is to be completed by the person with whom the child is living.
