Registration Guidelines

REGISTRATION GUIDELINES

 All persons registering must show identification.

If you are a single divorced or separated parent you must provide a copy of custody papers for the child's file.

If you do not live at the same address as your child, a "pre-affidavit statement" form must be completed by the person with whom the child is living. Call school for more information.

If you are not the parent, you must show legal custody documentation.

If you are not the parent and do not have legal custody call the school for more information.  A "Pre-affidavit statement" is required.

You must have the following documents with you at time of registration:

  • Proof of residency --Four documents, see attached.
  • Original immunization record for the child.
  • Proof of age- original birth certificate, baptismal certificate, hospital certificate, visa, or green card
  • Other Required Documents

Pre-Kindergarten students must be four by September 1st of the school year.  No exceptions.

Kindergarten students must be five by September 1st of the school year. No exceptions.

First grade students must be six by September 1st of the school year, No exceptions.
